Whether you’re searching for or residing in a rehabilitation or nursing home, HarmonyCare is with you every step of the way.
Your HarmonyCare Nurse Advocate Can:

  • Accompany you as you tour different nursing homes

  • Greet your loved one upon admission to the nursing home

  • Visit your loved one within the first week

  • Visit your loved one and attend the Care Plan meeting by the end of the third week

  • Visit your loved one and attend subsequent Care Plan meetings quarterly

  • Call the nursing home between visits to check up on your loved one

Detailed List of Services for Clients in a
Rehab & Nursing Home

FACILITY TOURS

(up to 4 hours)

Choosing the right nursing home is very important. That is why it is wise to tour more than one nursing home in your area so you can compare them to each other. Your HarmonyCare Nurse Advocate will accompany you on the tours to point out things that an untrained person might not notice and to help you evaluate the significance of the things that you do notice.

FACILITY ARRIVAL

(up to 2 hours)

Arriving at a nursing home can be stressful, especially if it is for the first time. The environment, medication, and personal care equipment may not be the same as in the hospital. The staff, policies, and schedules for various daily events will be different as well. Your HarmonyCare Nurse Advocate will come to the nursing home when you or your loved one arrive to make sure that everything goes smoothly, is explained clearly, and that you or your loved one are comfortable.  

ONE WEEK VISIT

(up to 1.5 hours) 

Your HarmonyCare Nurse Advocate will visit you or your loved one within the first week of arriving at the nursing home to ensure that everything is going well and will be able to speak to the nursing home staff if anything needs further attention. A confidential status update of the visit could be sent to a family member upon request.  

THREE WEEK CARE PLAN MEETING VISIT 

(up to 1.5 hours)

By the end of the third week, the nursing home will invite you or your loved one to an interdisciplinary meeting to review the plan of care based on the assessments done up until that point and to discuss discharge planning if applicable. The information presented is often overwhelming. Having your HarmonyCare Nurse Advocate (HCNA) attend the meeting to explain everything is very helpful. It could also prevent a discharge from being planned before you or your loved one are really ready to go. Your HCNA will visit with you or your loved one before the meeting to find out if there are any items that need to be addressed by any of the nursing home attendees. A confidential status update of the visit could be sent to a family member upon request.  

 

QUARTERLY CARE PLAN MEETING VISIT

(up to 1.5 hours)

For clients remaining in the nursing home longer than 100 days, interdisciplinary meetings will be scheduled quarterly for a review of the plan of care and for any new items to be discussed. Having your HarmonyCare Nurse Advocate (HCNA) attend the meeting to explain everything is very valuable. Your HCNA will visit with you or your loved one before the meeting to find out if there are any items that need to be addressed by any of the nursing home attendees. A confidential status update of the visit could be sent to a family member upon request.  

FACILITY CALLS

(up to 30 minutes)

Your HarmonyCare Nurse Advocate will call the nursing home for updates on you or your loved one or to resolve any issues that may come up in between visits. A confidential status update of the call could be sent to a family member upon request.  

"JUST BECAUSE" VISIT (NURSING)

(up to 1 hour)

Your HarmonyCare Nurse Advocate will visit you or your loved one at the facility to check on any medical concerns or to deal with situations that are best dealt with in person. A confidential status update of the visit could be sent to a family member upon request.

 

"JUST BECAUSE" VISIT (SOCIAL)

(up to 1 hour)

HarmonyCare will arrange for you or your loved one to receive social visits in the nursing home from our non-nursing advocate staff. They could spend the time doing any activity that you or your loved one will enjoy. We have friendly, compassionate staff and you or your loved one will look forward to their companionship. A confidential non-medical status update of the visit could be sent to a family member upon request.  

OUTINGS (NURSING/SOCIAL)

(up to 2 hours)

Your HarmonyCare Advocate staff will accompany you or your loved one to outside appointments (medical, dental, etc.) and on any other outings (shopping, salon, etc.). You or your loved one will look forward to these trips and will cherish the memories. A confidential update of any medical appointment could be sent to a family member if requested (as well as pictures of the hair and nails after a trip to the salon).

HOSPITAL VISIT (SCHEDULED)

(up to 1.5 hours)

Your HarmonyCare Nurse Advocate will visit the hospital if you or your loved one are sent there from the nursing home. The value of seeing a familiar, trusted face of someone who could speak to hospital staff and explain things clearly is enormous.  The benefit of the hospital staff seeing an involved family representative is well known. This visit is also extremely helpful in ensuring continuity of care upon admission to the hospital and upon return to the facility or to home. HOSPITAL VISIT ("STAT") 

(up to 2 hours)

A HarmonyCare Nurse Advocate (HCNA) will go to the hospital as soon as possible after we find out that you or your loved one have been sent there from the nursing home. This lets the hospital know that there is an involved family in this patient's care, which is so important.  This also allows the HCNA to update concerned family members upon request who may not be able to get to the hospital.  Please see the "Hospital" page for a complete list of the services we offer there.

HOME MODIFICATIONS

If modifications are needed at home prior to discharge from the nursing home to improve accessibility, HarmonyCare will know just who to call to get the work done neatly, on schedule, and for a fair price, all to your exacting specifications.

DISCHARGE TO HOME COORDINATION

(up to 1.5 hours)

Your HarmonyCare Nurse Advocate will visit the nursing home and the home setting to confirm that everything that is needed (shower chair, safety bars, medication, etc.) to ensure a safe transition to home is ready and in place.
