HOSPITAL SERVICES
From arrival at the hospital until time of discharge, HarmonyCare puts experience and knowledge on your side.
Your HarmonyCare Nurse Advocate Can:

  • Visit the hospital to advocate for your loved one of any age and to explain to you in simple terms what the medical staff is saying. Your loved one will enjoy the companionship too.

  • Arrange for home modifications if necessary before your loved one is discharged from the hospital

  • Discharge to home coordination

Clients in the Hospital

HOSPITAL VISITS - NURSING  ("STAT")     

(Up to 2 hours)

A HarmonyCare Nurse Advocate (HCNA) will go to the hospital as soon as we find out that you or your loved one have been sent there. This lets the hospital know that there is an involved family in this patient's care, which is so important.  This also allows the HCNA to update concerned family members upon request who may not be able to get to the hospital.

HOSPITAL VISITS - NURSING (SCHEDULED)         

(Up to 1.5 hours)

Your HarmonyCare Nurse Advocate will schedule a visit to the hospital if you or your loved one is sent there. The value of seeing a familiar, trusted face of someone who could speak to the medical staff and explain things to you clearly is enormous. The benefit of the hospital staff seeing an involved family representative is well known. This visit is also extremely helpful in ensuring continuity of care upon admission to the hospital and upon return home or to any other setting. A confidential status update of the visit could be sent to a family member upon request. 

HOSPITAL VISITS - (SOCIAL)  

(Up to 1.5 hours)

HarmonyCare will arrange for you or your loved one to receive social hospital visits from our non-nursing advocate staff. They could spend the time doing any activity that your loved one will enjoy. We have friendly, compassionate staff and your loved one will look forward to their companionship. A confidential non-medical status update of the visit could be sent to a family member upon request.  

HOME MODIFICATIONS

If modifications are needed at home to improve or maintain accessibility and safety, HarmonyCare knows just who to call to get the work done neatly, on schedule, and for a fair price, all to your exacting specifications.

DISCHARGE TO HOME - COORDINATION   

(Up to 2 hours)

Your HarmonyCare Nurse Advocate will visit the hospital and the home setting to confirm that everything that is needed (shower chair, safety bars, medication, etc.) to ensure a safe transition to home is ready and in place.
