LIVING AT HOME
HarmonyCare is your eyes, hands, and heart across the miles. We bring our experience home, ensuring your loved one is cared for as one of our own.
Your HarmonyCare Nurse Advocate Can:

  • Visit the home to check on your loved one and initiate a Tele-Medicine Physician visit if requested. Your loved one will enjoy the companionship too.
  • Ensure that the Home Aide is providing services as expected

  • Accompany your loved one to medical appointments, on shopping trips, or other outings

  • Deal with the home care and/or medical supply agency to resolve all issues timely

  • Arrange for home modifications if necessary to maintain accessibility and safety

  • Visit your loved one if sent to the hospital to be a familiar face and to ensure continuity of care

Detailed List of Services for Clients Living at Home
(Alone or With Home Care)

HOME VISITS (NURSING)

(up to 1.5 hours)                             

Your HarmonyCare Nurse Advocate (HCNA) will visit the home to check on you or your loved one. If there is a medical concern and getting out to the doctor is difficult, your HCNA could initiate a Tele-Medicine Physician visit right there in the comfort of the home.  If you or your loved one are receiving home care and you would like to be certain that services are being provided as expected, your HCNA could make unannounced visits at random times to reassure you. A confidential status update of the home visit could be sent to a family member upon request.  

HOME VISITS (SOCIAL)

(up to 1.5 hours)

Your HarmonyCare Nurse Advocate could arrange for you or your loved one to receive social home visits from our non-nursing advocate staff. They could spend the time doing any activity that you or your loved one will enjoy. We have friendly, compassionate staff and you or your loved one will look forward to their companionship. A confidential non-medical status update of the visit could be sent to a family member upon request.  

OUTINGS (NURSING/SOCIAL)

(up to 2 hours)

Your HarmonyCare Advocate staff is available to accompany you or your loved one to outside appointments (medical, dental, etc.) and on any other outings (shopping, salon, etc.). You or your loved one will look forward to these trips and will cherish the memories. A confidential update of any medical appointment could be sent to a family member if requested (as well as pictures of the hair and nails after a trip to the salon). We could also send you an alert when follow up appointments need to be scheduled or we could make them for you and remind you of them, if requested.

DEALING WITH HOME CARE/NURSING SUPPLY AGENCIES           

(up to 1.5 hours monthly)

We know how stressful it could be to deal with the agencies providing care at home, whether it is the aide who did not show up for their shift or finding out that needed medical supplies are running low. One call to HarmonyCare will get the ball rolling in taking care of your problem. We will of course keep you updated with whatever progress we make and the hopefully positive resolution. HarmonyCare is also able to see to it that anything that has to be done at set intervals (ordering supplies, getting the monthly home care schedule, etc.) is handled timely and responsibly.

HOME MODIFICATIONS

If modifications are needed at home to maintain or improve accessibility and safety, your HarmonyCare Advocate will know just who to call to get the work done neatly, on schedule, and for a fair price, all to your exacting specifications.

HOSPITAL VISITS (SCHEDULED)

(up to 1.5 hours)

Your HarmonyCare Nurse Advocate will visit the hospital if you or your loved one is sent there from home. The value of seeing a familiar, trusted face of someone who could speak to the hospital staff and explain things clearly is enormous. The benefit of the hospital staff seeing an involved family representative is well known. This visit is also extremely helpful in ensuring continuity of care upon admission to the hospital and upon return home or to any other setting. HOSPITAL VISITS ("STAT") 

(up to 2 hours)

A HarmonyCare Nurse Advocate (HCNA) will come to the hospital as soon as possible after we find out that you or your loved one have been sent there. This lets the hospital know that there is a caring family involved in the care, which is so important.  This also allows the HCNA to update concerned family members upon request who may not be able to get to the hospital.
